How to verify your insurance benefits

1 Call the member service phone number on the back of your insurance card.

2 Tell them you want to verify massage benefits. Keep a record of the date and who you spoke with.

3 What is my effective date for insurance coverage?

4 Is my LMT (Heather Carver) in network?

5 What are my benefits?

6 What is my deductible?

7 Has it been met? If not, how much is left?

8 What is my co-pay/co-insurance?

9 Is there a maximum dollar amount covered per year?

10 Is there a maximum number of visits covered per year?
